Abstract

The foamy gland pattern (FGP) may impart to pancreatic ductal adenocarcinoma (PDA) a deceptively benign appearance. Thus, its diagnosis on FNA is challenging. A case of PDA with FGP, that was suspected on Diff Quik smears, diagnosed on cell-block preparation and confirmed by ancillary stains and molecular techniques, is here reported. The diagnostic interpretation of foamy atypical cells on direct smears may benefit from cell block preparation and from ancillary techniques. Diagn. Cytopathol. 2013. © 2012 Wiley Periodicals, Inc.

Abstract

INTRODUCTION:

Management of postoperative esophagojejunal anastomotic leakage after total gastrectomy represents a very challenging event. Surgical repair is difficult, and conservative treatment can predispose to more severe complications. Endoclips and self-expanding stents are useful endoscopic therapeutic options but present some drawbacks. The Over-The-Scope-Clip (OTSC) system has been shown to be appropriate to close acute small gastrointestinal perforations, but its use in the treatment of chronic leakage remains controversial.

CASE SERIES:

The present series reports three consecutive chronic esophagojejunal anastomotic leaks successfully treated with OTSC. In all cases, clip application was simple, safe and effective, without early and late complications.

DISCUSSION:

The OTSC system may represent a new therapeutic option in the management of postoperative esophagojejunal anastomotic leakage.

Efficancy of capsule endoscopy in the follow up of Crohn’s disease.

Wireless capsule endoscopy (WCE) represents an advanced for small bowel imaging.

suspected Crohn’s disease represents one of the main indication of vce where it’s usefull

in terms of site and localizzation of the disease, drugs responsivness and in the follow up

of IBD patients, optimizing clinical and therapeutic work up.
We studied with vce a patient of yrs, with ileal Crohn’s disease in mild activity,

inflammatory tipes, without obstructive symptoms.
The current therapy was 5ASA not full dosage, also the patient’s compliance

is not very well. Not significant modifications in terms of bioumoral flogosis levels,

and protein spectrum. Anemia with diarrea and abdominal pain is the only

symtoms referred at hospital admission.
We performed a colonoscopy with ileoscopy that confirmed the diagnosis of

distal ileum Crohn’s disease; the vce following it was usufull to evaluate possible

proximal ileum and dejunum localization and also if is needed to modificate

the patient’s therapy the small bowel study with vce confirmed the ileum

( proximal and distal ) site, without any other localization and it was efficacy

to optimaze the current therapywith budesonide 9 gr 7 die plus 5 ASA 2,4 gr / die
